Poor little puppy! She's obviously not had the best start in life, she was not socialised as she should have been and taken from her mother and the litter far too early.
She is far too young for you to be putting her outside on her own. You need to stay outside with her when you put her outside - every hour or so - there are lots of tips on house training here on the forum.
Go outside with her, play with her out there. Give her lots ad lots of attention and make outside a really positive place for her to be.
