Yes you can use vinegar. The usual dilution rate is 1 part vinegar to 1 part water. Some people recommend cider vinegar, some say distilled white vinegar. Personally I don't think it matters.
Moisten cotton balls and swab as far down the ear canal as you can reach. I don't recommend flushing the ears because you can't see the ear drum to tell if it may be perforated. You don't want to flush infected material into the middle ear.

You can use also Hydrogen Peroxide on the ears only with a q-tip. Neosporin also works but you have to make sure that your dog does not scratch his ear then lick his paw.

I know how you feel, one day my dog stopped playing and he would just lie down on the floor. I did not think there was something wrong with him but it turned out that he was sireosly ill. what you should do is just ask diferent vets, and I promis that at least one of them will find a solution.
